Rezumat | ||||||
Background. DM complicates up to 10% of all pregnancies. Out of these, 0.3% are patients with type 1 diabetes. Recent advances in insulin therapy have increased the number of available options to the obstetrical-endocrinology team to complete the pregnancy in a viable way for the fetus. Objective of the study. Assessment of pregnancy and birth behavior in pregnant women with type I diabetes complicated until the end of pregnancy when there are signs of danger to the life of the pregnant woman. Material and methods. Evaluation of clinical, laboratory and instrumental outcomes in pregnant women with insulin-dependent diabetes mellitus. Results. Clinical case: Patient X, 21 years old, hospitalized with dg. Pregnancy 25 weeks GI NI. DM Type I complicated with chronic renal failure, nephropathy, and hydronephrosis gr. III. Against this background, the pregnancy occurred with physiological evolution up to 25 weeks, followed by the aggravation of the diabetes (CR 229, UR 22). A multidisciplinary council (nephrologist, urologist, gynecologist, and endocrinologist) was held on the evolution of the pregnancy in favor of its preservation. As a result, a plan of conduct was determined which led to an improvement in dynamics (CR 135, UR 12). At 36 weeks, in accordance with the plan, the pregnancy was completed by caesarean section. Conclusion: The multidisciplinary approach of a pregnant woman with aggravated extragenital pathology allows the development of a complex interdisciplinary behavior with obtaining optimal conditions for the favorable completion of the pregnancy for both mother and child. |
