Background: ETC (early total care) involves definitive surgical stabilization of all long-bone fractures during the early phase of treatment (24–48 h). Objective of the study. To assess the outcome of early surgical treatment in a polytrauma patient. Material and Methods. Patient Y, 25 years old, suffered a road accident as a driver. He was transported urgently to the hospital in the yellow area of the Emergency Medicine Department. He was conscious, oriented in time and space, in a severe but hemodynamically stable condition, the upper left limb immobilized on the scarf and the lower right on the splint. The patient was investigated by the multidisciplinary team according to ATLS principles. Results: The established diagnosis was: Polytrauma. Associated trauma. Trochanteric fracture on the right Evans V. Fracture of the pubic and sciatic bone on the right without displacement. Acetabular fracture on the right without displacement. Scapular fracture on the left. Neer left humerus fracture with 4 parts. VII rib fracture on the right without moving. Lung contusion on the right. Heart contusion. TCCI, Cerebral contusion. Posttraumatic HSA. Soft tissue contusion of the head. ISS – 25 points. He was clinically and radiologically investigated. Thus, the decision was to hospitalize him for surgical treatment. After 6 hours from the hospitalization, a surgery was performed, and after another 24 hours, two more interventions in one stage, the patient’s condition being of stable average severity. Finally, the patient was discharged more than 15 days after admission, without further complications. Conclusion In hemodynamically stable and young patients, the definitive fixation of all fractures (ETC) is applied from the beginning. |
