Background. The Republic of Moldova (RM) has a population of about 4 million people. It ranks among the firsts in the world in terms of stroke incidence. In our country, every hour a person suffers a stroke. Objective of the study. Review of the current situation of stroke management and evolution of services in this field over the last years in RM, in order to develop an updated clinical approach and promote research for quality improvement of these services. Material and Methods. A retrospective study was performed on stroke subjects in country's hospitals, based on the RES-Q and the National Bureau of Statistics. Searched for articles on PubMed, with the terms: "stroke", "thrombolysis", "thrombectomy", "management" and filters: 2020-2021, 18+ years, English. 2351 publications were selected: 91 meta-analyses, 62 reviews and 23 systematic reviews. Results. The health status of RM’s population is one of the most unfavourable in the entire European area. Currently, there are only 3 hospitals with Stroke units in Chisinau, 2 of them performing endovascular treatment. Only 4 regional hospitals have access to tomography, exclusively during the day. The first intravenous thrombolysis was performed in 2005. Since 2018, it’s also performed in 2 regional hospitals. In 2018, the first thrombectomy was performed in RM. The post-Stroke rehabilitation rate accounted for only 1% in 2020, because of the pandemic. A national Action Plan for the reorganization of stroke management system in RM for 2020-2030 was presented in 2019. Conclusion. Major advances in stroke management in our country occurred in the last years. However, there is still a difference in the quality of stroke care throughout our country and continuous efforts should be made to eliminate it in the future. |
