Rezumat

The present study aimed to assess the prevalence and management of periapical granuloma in an urban Moldovan population according to gender, age group and tooth type. The present study was based on a descriptive approach, which aimed to identify and quantify the demographic, medical and dental aspects observed between January and June, 2019 in a sample of patients from Ungheni town in relation to the chronic periapical pathology. Data were collected from clinical files containing medical and dental histories, dental exams and radiographic examination. Patients were informed about the long-term prognosis and a decision was made for conservative non-surgical treatment based on Ca Hydroxide. EDTA 17% sol. was used during the instrumentation of the root canal. Subsequently it was irrigated with 2.5% NaOCl sol. and 0.05% chlorhexidine sol. The root canals were dried using sterile paper points. Initially the root canals were filled with calcium hydroxide paste, which was changed every 2 weeks for 3 months. Finally the root canals were filled with gutta-percha points and AH 26 sealer using cold lateral condensatio technique and coronal seal was applied. In the clinical study 77 patients between the ages of 13 and 67 years were included. Among them 43% women and 57% men. 67% were from the rural area. The most common cause of chronic periodontitis for these patients was the complications of dental caries. Also, data from the medical records indicated the prevalence of acute exacerbation during March (25%), and April (29%). These results can be explained by the by decreased host defense during the mentioned months. The efficiency of the treatment based on Ca Hydroxide constituted 63%, reducing the periapical index (PAI) from the value PAI-4 to PAI-2. For the other 2 categories, when the lesion became larger or remained of the same size, the option for surgery was considered to be taken into account. 1. Although periodontitis is the most common infectious disease, it is treatable and can be prevented. 2. Reducing the incidence and prevalence of periapical lesions may reduce its associated systemic diseases and complications. 3. Apical granuloma has asymptomatic evolution; Rarely - do patients experience nociceptive sensations. Radiography is necessary ind iagnosing the pathology. 4. Monitoring the healing of periapical lesions is essential through regular examinations.
