Rezumat

Hysterectomy remains the gold standard in the treatment of benign gynecological pathologies when medical or minimally invasive surgical treatment has failed. It is one of the most common gynecological interventions and results in sterility, lack of menstruation and consequences in the sexual, psychological, hormonal and social sphere. The aim of the study is t determine the evolution of sexual function, vasomotor symptoms and psychological status in women of reproductive age after hysterectomy. A prospective observational study was performed. The study included 40 women, of reproductive age, who underwent hysterectomy with or without ovariectomy. Vasomotor symptoms, sexual function and psychological status were evaluated using the Green Climacteric Scale. The patients were questioned preoperatively, on the 10th-12th postoperative day, 6 months and 12 months after the intervention. The results of the study highlighted the fact that preoperatively the climacteric score was 12.7 with the prevalence of the psychological profile (total score 7.5 ±0.23) followed by somatic symptoms (total score 4.1 ±0.26). Symptoms such as nervousness prevailed in 70% and headaches in 72.5%. In the early preoperative period, the total Green score decreased to 11.7 due to the reduction of psychological (total score 6.25 ±0.19) and somatic (total score 3.45 ±0.19) symptoms. On the other hand, the increase of the total score of the vasomotor profile was highlighted. During this period, 75% of patients reported feeling tired, 72.5% hot flushes, 67.5% night sweats. At 6 months postoperatively, the total climacteric score registered an increase of 14.6. Anxiety symptoms predominated, such as feeling nervous (in 77.5% of cases) and sleep disturbances (in 72.5%). The total score of the somatic profile was 3.95 ±0.37 and the predominant symptom was headache (82.5 %). Vasomotor profile score (2.2 ±0.17) and loss of sexual interest (total score 0.67 ±0.76) also increased. At 12 months postoperatively, a total climacteric score of 15.47 was noted, also with the predominance of psychological symptoms (8.3 ±0.28) followed by somatic symptoms (3.67 ±0.33). Vasomotor profile score increased (total score 2.52 ±0.05) and loss of sexual interest (total score 0.97 ±0.99). The results of our study show that hysterectomy can affect sexual function, the psychological state as well as lead to the appearance of climacteric syndrome with the onset of vasomotor symptoms during the first postoperative year.
