Background. The constrained knee prosthesis has the basic indication in revision arthroplasty, but from the latest literature, result that it has its place in primary total knee arthroplasty in cases of knee osteoarthritis associated with major deformities. Objective of the study. Evaluation of the method of surgical treatment with the use of the constrained prosthesis in the difficult primary total knee arthroplasty. Material and Methods. Our study is based on the surgical treatment, using the constrained knee prosthesis in the primary total knee arthroplasty, during 2019-2021, of 28 patients with gonarthrosis associated with severe deformities in varus or valgus, in the Prosthetic Orthopedics of Large Joints Department, Clinical Hospital of Traumatology and Orthopedics from Chisinau. Results. In our study, the following criteria were evaluated: the type of deformity - respectively valgus were 10 cases and varus 18 cases; degree of bone defect - 11 cases were with defects of the joint surfaces larger than 5 mm, where it was necessary to use augmentations; affected side - in 19 cases the right knee was affected and 9 cases the left one; the F/M ratio was 4/1; the mean age of the patients was 65 years; average duration of the intervention - 140 minutes; in 5 cases a lateral parapatellar approach was performed, of which 2 cases with osteotomy of tibial tuberosity; complications - 2 septic cases resolved by debridement and 1 case with periprosthetic fracture. Conclusion. Osteoarthritis of the knee progresses rapidly, leading to severe deformities and significant bone defects of the joint surfaces. These cases are indications for the use of the constrained prosthesis in the primary total knee arthroplasty, having promising results |
