The incidence of pemphigus vulgaris varies depending on clinical and epidemiological profi le in different regions of the world. The bullous and ulcerative-erosive changes of skin and the mucous membranes are the hallmarks of the disease. The pemphigus vulgaris pathology occupies the main places among the leading causes of the invalidity in the dermatological diseases, requiring the immunosuppressive therapy. We present a clinical case. A patient of 65 years with pemphigus vulgaris. The tissues were examined histologically with hematoxylin-eosin and picrofuxin staining, fi nding the systemic damage of internal organs in this pathology. Microscopically it was observed: keratinocytes with a rounded contour, separated by the supra-basilar intraepidermal acantolysis; intravesicular necrotic keratinocytes, the vacuolar dermatitis of the interface in dermo-epidermal junction; the foci of the infl ammatory reaction with lymphocytes, sometimes eosinophils and neutrophils and the sclerosis sectors in the internal organs. Histological fi ndings suggest that pemphigus vulgaris is a disease of the whole body, which can manifest a rich symptomatology, which speaks for that the disease is not strictly dermatological. This involves a vicious cycle of systemic degenerative and decompensated changes with an irreversible character, which increases mortality of pacients. |
