Расстройства вестибулярной функции у детей приводят к нарушениям не только восприятия положения и перемещения тела в пространстве, двигательной координации, но и базовых механизмов сенсорной интеграции. Тактильные, проприоцептивные, вестибулярные сигналы от собственного тела являются основой для приобретения и накопления сенсорного опыта, а также формирования образа тела. Признаки несостоятельности сенсорной интеграции проявляются не только расстройствами регуляции мышечного тонуса, координации, мелкой моторики, но обнаруживаются у детей с нарушениями формирования речи, проблемами школьного обучения и поведения. Дети и подростки с такими нарушениями – значительная часть контингента пациентов детского невролога. К числу приемов, позволяющих объектив-но выявить признаки вестибулярной дисфункции, относится метод вестибулярных миогенных вызванных потенциалов (ВМВП). При стимуляции уха громким звуком и записи сокращения m. sternocleidomastoideus можно оценить функцию нижнего вестибулярного нерва и вестибулоспинального тракта, саккуло-шейный рефлекс. Нарушение или замедле-ние ВМВП может служить маркером вестибулярной дисфункции. Цель исследования — оценка возможностей ВМВП в неврологической практике для диагностики вестибулярных расстройств у детей и подростков. Методом ВМВП обследованы 150 пациентов в возрасте от 1.5 до 16 лет, пришедших на прием к неврологу с различными жалобами, наиболее час-тыми из которых были задержка развития речи (44%) и проблемы обучения и формирования школьных навыков (20%). ВМВП в ответ на звуковую стимуляцию (щелчки) регистрировали на аппарате Нейро-МВП-4 (Нейрософт, Иваново). Щелчки 0.5 мс, 130 дБ УЗД предъявляли через наушники. От 4 до 25 стимулов усредняли в 5-15 сериях с последующей суперпози-цией для оценки воспроизводимости ответов. Оценивали латентности волн Р13 и N23 ВМВП, регистрируемых от m. sternocleidomastoideus на стороне предъявления звуков. Тоническое напряжение мышцы обеспечивали поворотом головы в сторону. Определены значения латентности волн ВМВП «в норме»: P13– 10.3±0.9 мс, N23– 15.8±1.8 мс (m±sd), различия для правой и левой стороны незначимы. Замедление проведения по путям саккуло-шейного рефлекса – превышение латентности волны Р13 ВМВП значения 12 мс (98-я перцентиль) – в клинической группе выявлено в 30% случаев. В большинстве случаев нарушения были латерализованы. Выявление скрытой вестибулярной дисфункции позволяет подобрать оптимальные комплексы стимульных воздействия и коррекционно-развивающих методик для реабилитации детей и подростков с данными неврологическими расстройствами. Disorders of vestibular function in children lead to violations of not only the perception of the position and movement of the body in space, motor coordination, but also the basic mechanisms of sensory integration. Tactile, proprioceptive, vestibular signals from one's own body are the basis for acquiring and accumulating sensory experience, as well as creating the image of one's body. Signs of the invalidity of sensory integration are found not only in violations of the regulation of the muscular tone, balance, coordination, fine motor skills, but often are also manifested by disorders of speech development, problems of schooling and behavior. Children and adolescents with these disorders form a significant part of patient of neurologist. Among the methods that allow objective revealing the signs of vestibular dysfunction, the method of vestibular evoked myogenic potentials (VEMP) applies. When stimulating the ear with a loud sound and recording the contraction of the sternocleidomastoid muscle, one can evaluate the function of the lower vestibular nerve and vestibulospinal tract, the sacculo-cervical reflex. Violation or retardation of VEMP can serve as a marker of vestibular dysfunction. The purpose of the study is to evaluate the capabilities of the VEMP in neurological practice for diagnosis of vestibular disorders in children and adolescents. A total of 150 patients aged from 1.5 to 16 years old who attended a neurologist with various complaints, the most frequent of which were delayed speech development (44%) and learning problems and the disorder of the formation of school skills (20%) were examined by the method of VEMP. The VEMPs were recorded in response to sound stimulation (clicks), on the Neuro-MEP-4 (Neurosoft, Ivanovo). Clicks of 0.5 ms duration were presented through headphones with an intensity of 130 dB SPL.The number of averaging was from 4 to 25 in 5-15 series with the subsequent superposition to evaluate the reproducibility of the responses. We estimate the latency of the waves of P13 and N23 of the VEMP, registered from m. sternocleidomastoideus on the side of presenting sounds. Tonic tension of the muscle was ensured by a turning of the head to the side. The “normative” latency values (m ± sd) for the waves of the VEMP were10.3 ± 0.9 ms for P13, and 15.8 ± 1.8 ms for N23. The slowing down of the path of the sacculo-cervical reflex – the excess of the latency of the P13 wave of the 12 ms value (98th percentile) – in the clinical group was revealed in 30% of cases. In most cases, the violations were lateralized. The detection of hidden vestibular dysfunction allows to select the optimal complexes of stimuli and correcting techniques for the rehabilitation of children and adolescents with these neurological disorders.
