Introducere. Managementul clinic al cancerului renal este complicat defaptul că acesta este foarte rezistent la chimioterapie şi radioterapie, iar imunoterapia a demonstrat unpotenţial semnificativ de toxicitate. Metoda chirurgicală ramâne unica metodă eficientă, iar nefrectomia radicală este standardul de aur în tratamentul cancerului renal. Scopul lucrării. Determinarea metodei de tratament aplicate bolnavilor cu cancer renal, în dependenţă de stadiul clinic şi gradaţia histopatologică. Material şi metode. Studiu retrospectiv. În calitate de material de studiu a servit documentaţia medicală (fişele medicale şi protocoalele de operaţie) a 145 de pacienţi cu diagnosticul de cancer renal în stadiile T1-T4, internaţi în secţia Urologie al Institutului Oncologic, în perioada anului 2015. Rezultate. În rezultatul investigatiilor a 145 de pacienti din lotul de studiu, la 24 pacienţi (16,55%) a fost aplicată nefrectomia parţială, iar la 121 pacienţi (83,44%) – nefrectomia radicală. Nefrectomia parţială a fost abordată în cazul pacienţilor din stadiul I, T1aN0M0 (cu dimensiunile tumorii <4 cm), iar metoda radicală a fost abordată pentru tumorile de dimensiuni de la 4 la 7cm – stadiul I, T1bN0M0 şi stadiile II, III şi IV. În conformitate cu gradaţia histopatologică, în G3, de prima intenţie este nefrectomia radicală, iar stadiul G2 şi G1 de diferenţiere permite aplicarea ambelor metode de tratament chirurgical. Concluzii. Metoda de tratament de bază în managementul cancerului renal ramâne a fi tratamentul chirurgical. Alegerea tacticii chirurgicale depinde de stadiul clinic al pacientului şi gradaţia histopatologică.          

Introduction. Clinical management of kidney cancer is complicated by the fact that it is very resistant to chemotherapy and radiotherapy, and immunotherapy has shown a significant potential of toxicity. The surgical method remains the only effective method, and radical nephrectomy is the gold standard in the treatment of kidney cancer. Objective of the study. Determination of the treatment method applied to patients with kidney cancer, depending on the clinical stage and the histopathological gradation. Material and methods. A retrospective study. Medical documentation (medical records and surgical protocols) of 145 patients with the diagnosis of kidney cancer in the T1-T4 stages, that were hospitalized in the Urology Department of the Oncology Institute during the year 2015 served as study material. Results. After studying the result of the investigations of 145 patients from the study group, the following were revealed: in 24 patients (16.55%) partial nephrectomy was applied, and 121 patients (83.44%) – underwent radical nephrectomy.Partial nephrectomy has been made in the patients with stage I, T1aN0M0 (tumor size <4 cm); and the radical method has been made in cases of tumors with 4-7cm in size, stage I, T1bN0M0 and stages II, III and IV. According to the histopathological gradation, in differenciation grade G3 – the first intention is radical nephrectomy, but stage G2 and G1 of differentiation allows the application of both methods of surgical treatment. Conclusions. The main treatment method in the management of kidney cancer remains the surgical one. The choice of surgical tactics depends on the clinical stage of the patient and the histopathological gradation.
