Rezumat | ||||||
Background. Nonparasitic congenital cysts of the liver have a low frequency during childhood. The first mention of this condition belongs to Brodie (1952), and the first full description of a clinical observation belongs to Michel (1856). Objective of the study. Analysis of patients with nonparasitic cysts of the liver in order to improve the diagnosis and the results of treatment in this category of patients. Material and Methods. During 50 years at Natalia Gheorghiu DPSOA, 31 observations of nonparasitic cysts of the liver have been recorded. This study shows that in 18 cases the right lobe was affected, in 3 - both lobes, in 10the left lobe. In 22 cases the female sex were affected, in 4 patients the contents of the cysts were not suppurated. In 3 patients we diagnosed multiple hepatic cysts, small in size (3-5 mm), disseminated in the right lobe of the liver. The diagnosis was confirmed by abdominal ultrasound, CT with angiography. Results. In all cases, surgery was performed. A patient was found with a cystic tumor 6.0x5.0cm, with a non-purulent lactescent liquid content. The cyst membrane was transparent, thin. A partial cystectomy was performed, completely removing the membrane. Doxycycline filling and local drainage. The after surgery evolution was favorable. Histopathological conclusion: solitary, nonparasitic cyst. The patient was discharged on the 12-th day after surgery and assessed 1-3 months after surgery by clinical, paraclinical, imaging, biohumoral studies without recurrence. Conclusions. (1) Most of nonparasitic hepatic cysts are asymptomatic, constituting accidental findings. (2) The ideal cystectomy is a very difficult surgical procedure, with high risks of bleeding when the adherent membrane is detected by the liver parenchyma. |
