Rezumat | |||||||
Introduction. Vesicoureteral reflux in children is the retrograde passage of urine from the bladder to the ureter. The evolution of this pathological entity is extremely variable, from spontaneous resolution to the appearance of kidney scars and chronic kidney disease. The factors that contribute to the appearance of kidney scars are the severity of reflux, the frequency and severity of urinary tract infections. Purpose. Identification of clinical, biohumoral, imaging implications in the diagnosis and treatment of vesicoureteral reflux in children, especially in classical surgical therapy addressed to the given pathology. Material and methods. The study was conducted on a group of 519 children aged from newborn to 18 years. Females (70.5%) predominated over males (29.5%). Unilateral reflux - 357 (68.8%) patients, bilateral - 162 (31.2%). The study was performed according to the standard examination protocol, which included clinical examination, laboratory, imaging, urography, cystography, scintigraphy, urinary rhythm, cystoadaptometry, and cystoscopy. Results. In the vast majority of cases, VUR of I-III degree in young children was resolved conservatively following a medication according to a well-argued clinical-imaging, biohumoral scheme. 204 patients on 261 ureters were surgically resolved, using the Gregoir technique in 184, the Mob-ly technique in 70 and other technics – in 7 children. Conclusions. Vesicoureteral reflux of I-III degree without renal function disorder was resolved conservatively. In patients with VUR of II-III degree, the Gregoir technique remains the elected operation. In patients with reflux of IV-V degree and sinusoidal ureter, resection of the ureter in the lower 1/3 and ureterocystoneoanastomosis were performed. |
