Background. Malnutrition is a major risk factor of mortality and morbidity of children with chronic kidney disease (CKD), resulting from loss of protein, microelements and energy. The prevalence of malnutrition in children with CKD is 20–45%. Objective of the study. Presentation of a clinical case of a child with chronic kidney disease associated with malnutrition. Material and Methods. A clinical case describes 2.8-year-old male patient with a weight of 10.6 kg (P 3, Z-1.91); waist – 76 cm (P < 1, Z-4.52); BMI 18.35 (P 95, Z 1.63); WI-0.7; SI-0.84; NS-1.06. Pale, with dry skin, reduced turgor and elasticity. With presence of suborbital circles. The child is on renal replacement therapy for one year. Results. There are blood test changes with decreased hemoglobin (78 g/l), erythrocytes (2.6x1012/l) and hematocrit - 24.8%; urea - 36.6 mmol/l, and creatinine – 420 mcmol/l, total protein – 55 g/l, albumin - 36.8 g/l; ionogram shows hyperkalemia (6.1 mmol/l), hypernatremia (141 mmol/l) and hypermagnesemia, accompanied by decompensated metabolic acidosis. Analysis of urine detected moderate leukocyturia. Glomerular filtration – 11 ml/ min/1.73m2. Kidney ultrasound: right kidney - 41×28 mm, left kidney - 50×27 mm, right parenchyma – 9 mm, left – 10 mm, relatively increased echogenicity, right pelvis – 2 mm, left – 18 mm, ureter dilated throughout up to 13mm. Conclusion. In addition to the many complications of CKD in children, growth failure and malnutrition remain a clinically significant age-dependent problem, which requires a multidisciplinary approach of the patient, with the establishment of the right management and providing favorable development. |
