Background. Gastric cancer is one of the major problem in the field of medicine because of its incidence and poor prognosis. Due to late onset, it can be associated with complications: stenosis, hemorrhage, perforation, dysphagia, gastrointestinal fistulas, and invasion. Objective of the study. Presentation of a clinical case of a patient with confirmed morphopathological gastric cancer, associated with perforation and hemorrhage, who was treated in the Gastrology Department of the Institute of Oncology. Material and Methods. Anamnestic, clinical and paraclinical data were taken from the medical record. The patient was investigated endoscopically, radioscopically, morphohistopathological examination, electrocardiography, laboratory examination. The literature on similar cases has been studied. Results. Patient, 73 years old, hospitalized in the Gastrology department of IO in serious general condition, after treatment in the District Hospital. Patient urgently investigated with hemoglobin detection 24.00 g/l, erythrocytes 0.90x1012/L, hematocrit 0.80%. Hemodynamically stabilized patient with subsequent surgery for vital reasons in volume of subtotal palliative gastrectomy. Intraoperatively a neoplastic process of approximately 6x5x6 cm with central perforation was detected during on the small gastric curvature with passage on the anterior and posterior wall. Blood was attested in the omental bursa. In the liver two metastatic foci of approximately 1.5x2 cm each in segments VI and VII. It has been identified that the neoplastic process invades the mesocolon. Postoperatively with favorable clinical evolution, the patient was discharged on 11th day at home. Conclusions. Gastric cancer can have multiple serious complications that, if not corrected in time, aggravate the patient’s general condition, which is why these patients require a broad multidisciplinary medical approach. |
