Background. Liver cirrhosis (LC) is the most common cause of portal hypertension (PH). The cirrhotic patients remains a current topic that requires a management with wellestablished objectives. Cases of non-Hodgkin’s lymphoma associated with LC have been reported in the literature. Objective of the study. Revealing the importance of the complex approach of the decompensated cirrhotic patient and the establishment of the complex pathogenic argued treatment by presenting a case study. Material and Methods. A 45-year-old man was admitted to the Surgery Clinic No. 2 in May 2021. The diagnosis of decompensated LC was initially established. After additional examinations (including CT), the diagnosis of non-Hodgkin’s lymphoma (in addition to liver pathology) is suggested, highlighted by laparoscopy and confirmed by histopathological examination. Results. Several causes of PH reveal: CH, the mass effect of the lymph nodes with compression on the hepatic hilum, but also on the cervical portion of the TLD. TLD decompression, laparoscopic curing were performed. Intraoperatively, after removal of lymph nodes comprising TLD, it was observed dilatation by 5 mm on the terminal portion of TLD, with the restoration of drainage function. Early results: increased in diuresis from about 800 ml preoperatively to about 2.4 l / 24 h postoperatively, with supportive diuretic therapy, respectively the obvious regression of ascites syndrome. Conclusions. Non-Hodgkin’s lymphoma and LC, even if are separate diseases, they simultaneously potentiated the increase of PH and lymphocirculatory block at the central level, and the progression of ascites syndrome. The reactivation of lymphocirculation is pathogenetically argued. |
