Background. Tibial plateau fractures represent approximately 1% of the total fractures in the general population and 8% among the senile age population. Most of them are caused by road accidents and catatraumatisms, 90% of them being associated with different degrees of soft tissue injuries. Objective of the study. Appreciation of the clinical-imaging results of the surgical management by choosing different surgical approaches, types of orthopedic implants, reduction quality, stability of osteosynthesis and functional recovery. Material and Methods. Were analyzed 125 clinical cases: 70 women, 55 men. The classification of fractures according to Schatzker was used: of which type I-10 cases, II-35, III-15, IV-10, V-35, VI-20. Surgical management consisted of closed reduction in 15 cases: osteosynthesis with screws-10, external fixator-5 and open reduction with internal osteosynthesis in 110 cases. Results. Patients were evaluated clinically and imaging at a period of 6, 12, 24 weeks postoperatively, the functional result was assessed according to the Lysholm Knee Scoring Scale, obtaining an average result of 89 points. Bone healing was determined at an average period of 12 weeks. Various postoperative complications developed in 18 patients. The results were dependent on the approaches, the surgical techniques used, the type, the stability of the osteosynthesis, the precocity, the correctness of the functional recovery and the compliance of the patients. Conclusion. Favorable results, easier and fewer complications were observed in cases of individual approach to surgical management, optimal choice of approaches, implants, stable osteosynthesis, early and correct functional recovery. |
