Eventratiile gigante postoperatorii (EGP) sunt raportate în 0,35 – 1,5%. Scopul interventiei chirurgicale în EGP este - reintroducerea viscerelor în abdomen cu evitarea sindromului de compartiment si reconstructia defectului cu obtinerea unui perete functional si dinamic. Aceste aspecte prezinta dificultati mai mari în situatii de urgente medico-chirurgicale. Material si metode: Este expusa atitudinea medico-chirurgicala în 7 cazuri de EGP asociate cu sindrom ocluziv, selectate din 118 cazuri de eventratii postoperatorii, tratate în SC ”Sf.Arh.Mihail” în perioada 2016 – 2019. Toti pacientii au fost transportati în stationar cu EGP si suspectie de ocluzie intestinala acuta. Diagnosticul s-a bazat pe USG, R-grafia de ansamblu si R-grafia cu masã de contrast, baterii de teste. Rezultate: Tratamentul conservativ de reechilibrare volemica æi decompresia gastrica aplicate, au ameliorat în 5 cazuri starea pacientilor. Ulterior bolnavii s-au prezentat cu date de sindrom subocluziv aderential în sacul de eventratie, fiind create conditii de tratament chirurgical în urgenta amânata în termini de 7-8 zile, dupa o pregãtire speciala. In 2 cazuri de manifestari ocluzive persistente, interventia a fost de urgenta dupa o pregatire de 1-2 ore. De obicei intraoperator în EGP s-a constatat un sac multicameral æi aderente multiple, ce a facut dificila utilizarea sacului la închiderea cavitaåii peritoneale. În 2 cazuri de ocluzie acuta s-au evidentiat date de congestie a intestinului cu schimbari reversibile. Adezioliza extinsa nu s-a asociat cu complicaåii. Pentru plastia defectului s-a utilizat tehnica de substitutie - grefa de polipropilena 30*30 cm, amplasata intra - abdominal TENSION FREE, deschisã în spaåiul subcutanat. În toate cazurile abdomenul a fost drenat cu tub din silicon nr. 14, înlaturat la 4-6 zi. Morbiditate si mortalitate postoperatorie nu s-a constatat Concluzii: Atitudinea chirurgicala în eventratiile gigante cu sindrom ocluziv necesita diferetiere si individualizare. Refacerea peretelui abdominal prin metoda de substititie cu amplasare intra – abdominala a protezei sintetice poate fi o optiune în EGP asociate cu sindrom ocluziv atat în interventia amanata, cat si în cea urgenta în cazul schimbarilor morfologice reversibile intestinale.
Giant incisional hernia (GIH) are reported in 0.35 - 1.5%. The purpose of the surgery in GIH is - the reintroduction of the viscera in the abdominal cavity with the avoidance of the compartment syndrome and the reconstruction of the defect with the obtaining of a functional and dynamic wall. These aspects present greater difficulties in medical-surgical emergencies. Material and methods: The medical-surgical attitude is exposed in 7 cases of GIH associated with occlusive syndrome, selected from 118 cases of incisional hernia, treated in Clinical Hospital ""Sf.Arh.Mihail"" during 2016 - 2019. All patients were transported in hospital with GIH and suspected acute intestinal occlusion. The diagnosis was based on USG, simple X ray and X ray with contrast mass, laboratory tests. Results: Conservative treatment of volume rebalancing and gastric decompression applied, improved in 5 cases the condition of patients. Subsequently, the patients presented with data of subocclusive adhesion syndrome in the eventration sac, being created conditions for surgical treatment after 7-8 days in delayed emergency, in terms of 7-8 days, after a special preparation. In 2 cases of persistent occlusive manifestations, the intervention was urgent after a preparation of 1-2 hours. Usually a multicameral sac and multiple adhesions were found intraoperatively in GIH, which made it difficult to use the sac when closing the peritoneal cavity. In 2 cases of acute occlusion, data of congestion of the intestine with reversible changes were highlighted. Extensive adhesolysis was not associated with complications. For the plasty of the defect, the substitution technique was used - polypropylene graft 30 * 30 cm, located inlay TENSION FREE, open in the subcutaneous space. In all cases the abdomen was drained with silicone tube no. 14, removed at 4-6 days. Postoperative morbidity and mortality were not found Conclusions: The surgical attitude in giant eventrations with occlusive syndrome requires differentiation and individualization. Restoration of the abdominal wall by the method of replacement with inlay location of the synthetic prosthesis may be an option in GIH associated with occlusive syndrome in both delayed and urgent intervention in case of reversible intestinal morphological changes.
