Background. The association of pelvic and urinary tract injuries varies from 4.4% to 17.26% of all associated lesions, they are a complex problem in emergency surgery. Objective of the study. To determine the surgical tactics in the treatment of pelvic fractures with associated urinary tract injuries. Material and Methods. Retrospective study, 59 patients: male – 55(93,22%), female – 4(6,78%), including a pregnant woman in the third trimester of pregnancy. Mean age – 41,34±21,66 years. The trauma mechanism: traffic accident – 47(79,66%), catatauma – 9(15,25%), massive wall collapse – 3(5,08%). ISS=33,52±0,13. Results. Urinary bladder repair and pelvic ring stabilization with external device was performed in all cases. Postoperative complications were: epicystostomy insufficiency – 1(1,69%), soft tissues inflammation in the area of fixation elements– 4(6,78%). Average length of hospitalization period – 49,62±17,27%. On discharge, all patients moved independently. Longterm results were evaluated in 48 cases: good – 30(67,64%), satisfactory – 16(27,45%), unsatisfactory – 2(4,9%). Conclusion. The preliminary results of treatment patients with associated lesions of the pelvic bones and urinary system indicates that adequate and early surgical correction of the pelvic ring with an external fixation device and urinary bladder repair is possible in almost all cases. |
