Background. Laparastoma is used in treatment of abdominal trauma and secondary peritonitis. But this has a high risk of evisceration, enterocutaneous fistulas, either and difficulties of care. This shortcomings generated the necessary of utilisation of some methodes of temporary closure of abdomen Objective of the study. The analyse of some methods of temporary closure of abdomen used in patients operated with mesenteric ischemia. Material and Methods. There were analysed temporary methods of closure of abdomen in 52 consecutive cases of mesenteric acute ischemia. The mean age of patients varied from 30 till 87 years old, the report M:F=1:1,17. Results. The volume of interventions: sectorial resections 25%, large resections 71,2%, without resections 3,8%. The temporary closure of laparastoma: a) rare sutures on aponeuroses and skin in 44,2%, b) VAC technique application - 13,5%, c) different procedures, using intraabdominal turundes of postsurgical wound or the both, combined with rare sutures in tisues - 42,3%. In case of wound tamponaded with gauze, the deffinitive closure was efectuated after the second revision in 30,8%, and in 9,6% after the 3-6 revision. In case of absence of gauze tampone and using VAC procedure, laparastoma was liquidated in first revision in 59,6%. Conclusion. For temporary closure of laparastoma in patients with mesenteric ischemia, can be used the VAC technique or the aplication of rare sutures on aponeuroses and skin. |
