Background. Gastroesophageal reflux disease (GERD) continues to occupy the leading position among gastroenterological diseases. The prevalence of GERD varies between 8 and 40%, in the Republic of Moldova - 17.4%. To date, not all the mechanisms of development of this pathology are known. Objective of the study. To study the peculiarities of morphofunctional changes of the esophageal mucosa in patients with different endoscopic forms of GERD. Material and Methods. Were included 104 patients, diagnosed clinically and endoscopically with GERD (nonerosive form - 27 and erosive form 77 patients). Age of patients 35-65 years (70 men and 34 women). In all patients in the study, fibroesophagogastroscopy was performed with esophageal bioptate sampling and histopatomorphological study, determination of nitric oxide (NO) metabolites in blood serum. Results. Inflammatory infiltrate and erosions of the esophageal mucosa were detected in 88.3% of cases (erosive form GERD). In both forms: hyperplasia of the basal layer (100%), elongation of the papillae (97.4%), vacuolation of cells (72.7%), acanthosis (27.3%), hyperkeratosis (42.9%), vascular changes (51 .9%), hemorrhagic infiltrate (42.6%). In 88.3% of cases, histolymphocytic infiltrate predominates, which is associated with interstitial hemorrhage (42.6%). Average value of stable NO metabolites in blood serum: 83.43 ± 0.7 0mol / l - non-erosive form of GERD, and erosive - 97.02 ± 0.4 μmol / l. Conclusion. 1. Histological examination of the esophageal bioptate found histological changes characteristic of the chronic inflammatory process, with typical local vascular changes, which correspond to the endoscopic form of GERD. 2. The level of NO metabolites in the blood serum correlates with the endoscopic. |
