Background. "Occult" HVBC may progress to clinical seropositive and seronegative form, characterized by the absence of HBsAg in the blood, the presence of summary anti-HBc / anti-HBs and positive HBV DNA in the titer <200 IU/ml in blood or liver tissue. These need to be differentiated from the "past infection". Objective of the study. to determine the anti-HBc / anti-HBs serological pattern and HBV DNA in children with hepatomegaly and liver disorders in which HBsAg is not detected to identify "occult" forms of hepatitis or "past infection". Material and Methods. The study included 35 children aged 1-12 years, hepatomegaly and cytolysis, who were examined for HBsAg, anti-HBs, summary anti-HBc (IE) and real-time HBV DNA (PCR) in the blood. Results. There was a risk of perinatal infection with HBV in only 2 children, all of whom were vaccinated against HBV at birth. In 5 (14%) cases, “occult” HVBC was found in seropositive form with summary anti-HBcor + in the absence of anti-HBs, HBV DNA in the blood being <10 μl / ml. In an adolescent with HBsAg negative, summary anti-HBcor + and HBV-negative DNA, anti-HBs at 72 μl / l and HBV-positive RNA were detected simultaneously. In one case, the status of "past infection" was determined with anti-HBs + and anti-HBcor summary +, but negative HBV DNA, against the background of acute HVG associated with nephroblastoma. Conclusion. "Occult" HVBC in children developed seropositive, with the risk of reactivation over time and the need for dynamic HBV DNA testing. |
