Introducere. Șocul cardiogen este o complicație severă a sindromului coronarian acut, cu o rată înaltă a letalității. Șocul cardiogen ca un eveniment instalat ar putea fi prevenit, constituind astfel obiectul cercetărilor în acest domeniu. Introduction. Cardiogenic shock is a severe complication of acute coronary syndrome (SCA) with a high death rate. Cardiogenic shock as an event could be prevented, so it is necessary to be continuously researched. Objective of the study. Study of the clinical-epidemiological aspects of cardiogenic shock in the early stages of IMA-STEMI and NSTEMI. Material and methods. A retrospective study. The research included 46 patients with the diagnosis of acute coronary syndrome complicated by cardiogenic shock, in conditions of emergency aid, in Chișinău during 1.01.2017-31.12.2018. Result. 28 patients were diagnosed with IMA STEMI and 17 patients – with IMA NSTEMI. 41% of patients had compensated shock, 35% – decompensated shock and 24% – irreversible shock. Risk factors that had been associated with: advanced age – 97.8%, female – 47.8%, hypertension – 43.5%, diabetes – 15.2%, pulmonary arterial hypertension – 13.1% and stroke – 6.5%. 26,1% patients had histories of previous infarction. Cardiogenic shock was developed within 6 hours from onset of infarction in 73.9%. Drug therapy was initiated in 100% and surgical procedures – 32.6%. Death occurred in 65%, of which in the first 24h – 67%. Conclusions. The prognosis of the patient with cardiogenic shock is determined by various factors: etiologies of shock, patient age, co-morbidity, early diagnosis and treatment etc. However, cardiogenic shock remains a major emergency, being a severe complication of IMA with very high mortality. |
