The authors present the case of a 15 year old boy, weight 83 kg, height – 168 cm, body mass index (BMI) - 29,4 kg/m2 (97.4 percentile), abdominal circumference (AС) - 98 cm ( 90 percentiles). The onset of excessive weight gain occurred at the age of 12, with rapid weight gain being linked to sedentary, hyperphagia, vicious eating habits. Following the laboratory and instrumental examinations performed the diagnosis of metabolic syndrome (abdominal circumference > 90 th percentile, triglyceride> 1.7 mmol / l, HDL cholesterol < 1.03 mmol /l) was established according to International Diabetes Federation, 2007 and congenital heart disease: bicuspid aortic valve; aortic valve stenosis (presidential gradient in maximum - 43 mmHg, maximum speed of 3.3 m/s, aortic valve insuffi ciency +, with signs of cardiac remodeling, epicardial adipose tissue – 6.1 mm and carotid intima-media thickness – 0,5 mm. In the context of association of metabolic syndrome with congenital aorticopathy has recommended the initiation of non-medication therapy, targeted on the component - obesity, cardiac supportive treatment and cardio-surgeon consultation to determine the necessity and opportunity of surgical treatment. |
