Background. Acute appendicitis of the newborn and infant has a special form of acute surgical abdomen, with a rare frequency at this age. Often diagnosed in the stage of generalized peritonitis. Objective of the study. is to present a newborn with gangrenous appendicitis complicated by perforating peritonitis. Material and Methods. anamnestic, clinical and paraclinical data were taken from the medical record. The patient was investigated by contrast-enhanced abdominal radiography. The literature on similar cases has been studied Results. Premature newborn, on the 17th day of life, was transferred from the RTI NN section with respiratory support, extremely serious condition, with clinical signs of intestinal occlusion: repeated vomiting, lack of stool, abdominal bloating. Radiological - data of intestinal occlusion. Emergency surgery was performed. The revision determined a conglomerate from the loops of the small intestine, adhered by fibrin, with severe discirculatory changes throughout, without peristalsis. When attempting to release the intestine, the gangrenous modified appendix is attested with two perforations at the tip. Appendectomy, terminal enterostomy was performed. Conclusion. Premature infants with acute appendicitis have an increased risk of two major gastrointestinal complications - UNE and spontaneous intestinal perforation, which are important causes of mortality in this group of patients. |
