Background. Acute appendicitis (AA) is the most common non-obstetric abdominal condition that requires surgical treatment during pregnancy, endangering the life of the mother and the fetus. AA develops from 1: 500 to 1: 6600 pregnancies, proportion: in the first trimester 19-36%, in the second 27-60%, in the third 15-33%. Objective of the study. Presentation of a rare clinical case - acute gangrenous appendicitis diagnosed in a pregnant woman during cesarean section. Material and Methods. 27-year-old pregnant, PI BI. She is urgently hospitalized in IMSP IMC with accusations of vaginal hemorrhage, and an indication to finish the pregnancy by emergency cesarean section with indication - Premature Detachment of Normal Inserted Placenta (PDNIP). The patient also had low-grade fever and a history of kidney infection during pregnancy. Results. During the operation, after extracting the fetus with an Apgar 8/8 score, a foul smell was felt in the abdominal cavity. The surgeon diagnosed AA and the appendicectomy was performed in parallel. Histopathological examination of the placenta confirmed the indication for emergency caesarean section - PDNIP, and the same examination of the extracted appendix: AA gangrenous, peritonitis and leukocyte omentitis, fibrino-necrotic focus. Conclusion. AA - rarely encountered in pregnant women and difficult to diagnose, in this clinical case was occasionally diagnosed against the background of another obstetric pathology-PDNIP. |
