Background. Myomas are benign monoclonal tumors, found in 1.6-10,7% cases during pregnancy. Ussually myomas are asymptomatic, but there is a high risk for obstetric complications related with the location and size of fibroids. Myomectomy during pregnancy isn`t a routine practice, is recommended just in some cases. Objective of the study. Reasonable management of such clinical cases in order to deliver the fetus and further preserve the maternal reproductive capacity, pregnancy outcomes after conservative myomectomy. Material and Methods. Anamnesis, clinical and paraclinical data were taken from the medical databases. The literature about similar cases has been studied. Results. A 40-year-old pregnant woman, at the term of 24 weeks of amenorrhea, was hospitalized, diagnosis: Multinodular uterine fibroids with necrosis of myomatous nodules, Hemorrhagic peritonitis. Due to pronounced pain syndrome, positive signs of peritonitis, conservative myomectomy was performed, intraoperative: hemorrhagic fluid with fibrin elements and necrotic tissue around 200 ml, multiple myomatous nodules with subserosal, intramural localization were found, a pediculated subserosal myomatous nodule-18cm with elements of necrosis and degenerative changes was removed. At 34 weeks, she was re-admitted with Prenatal rupture of the membranes, by caesarean section a babygirl was delivered. Conclusion. Fibroids during pregnancy need a particular attention, is important to establish a correct management in order to have good outcomes for both fetus and mother. |
