Background. The diagnosis of celiac disease is a dilemma of the medical community, both nationally and internationally, due to the multitude of atypical clinical manifestations, especially in children. The quality of life of these patients depends on early diagnosis. Objective of the study. To elucidate atypical clinical manifestations of celiac disease in children and to optimize diagnostic management. Material and Methods. The research was carried out within the PHCI Institute of Mother and Child, Integrated Specialized Consulting Department, by the multidisciplinary and immunoserological examination, ELISA immuno-enzymatic technique. Results. The patient, aged 6 years, complained of abdominal pain, early satiety, lack of upper incisors, facial rash resembling juvenile acne, for 2 years, and was referred to the gastroenterologist. Paraclinical tests revealed Hb100g/l, TPO-707 antibodies (N˂40). USG of the uterus and ovaries: uterine and ovarian hypoplasia, gr. II-III. Immunoserological examination found tTG, IgA-0.38 (0-0.3kU/L) and IgG-0.42 (0-0.3kU/L), IgA-0.36 (0-0.3kU/L) and IgG-4.6 (0-0.3kU/L). Diagnosis: Atypical Celiac disease. Autoimmune thyroiditis, compensated hypothyroidism. Uterine and ovarian hypoplasia, grade II-III. Iron deficiency anemia, gr.I. Chronic blepharoconjunctivitis. Dental caries. Conclusion. Atypical clinical manifestations require a multidisciplinary examination for early clinical diagnosis, thus preventing the irreversible impact on the development of the child with celiac disease. |
