Background. Arthroscopic tehnique Bankart for treatment of anterior shoulder instabilitity became a relevant method. Frequency of anterior shoulder instability is quite high, due to the anatomical and functional shoulder particularities. Objective of the study. Evaluation of results and the possibility for arthroscopic treatment for the patients with anterior shoulder instability. Material and Methods. The study is based on personal experience of treatment of 182 patients with arthroscopic stabilization of anterior shoulder instability between 2012-2021. Was performed the reinsertion of anterior-inferior part of the glenoid labrum and joint capsule using bioresorbable anchors (13 patients), metal (120 patients), transglenoidal unresorbable thread suture (49 patients). Results. After surgery the patients were evaluated at an interval of 12 to 24 months using the Rowe and Zarins score. From the study group we obtained excellent or good results in 166 patients (86.5%), unsatisfactory - 16 patients. All patients who previously practiced the sport after 4-6 months returned to their efforts. We found a negative result in 16 patients: 10 recurrent dislocations and 6 patients had symptoms of instability without dislocation. 10 patients underwent repeated arthroscopic Bankart surgical treatment, 6 patients underwent transposition of the coracoid process on the anterior part of the glenoid rim - Letarjet surgery. Conclusion. Arthroscopic treatment of anterior shoulder instability is a pretentious technique. The results depend on multiple factors, including age, technical errors, bone defects, number of dislocations, type of anchors, the presence of Hill-Sachs lesion and the pre-operatory bone geometry |
