Background. The results of the treatment of unstable pelvic ring fractures in women in 11-64% of cases are unsatisfactory. There are sporadic publications about patients with pelvic instability after birth. Postpartum women ignore pain in the lumbo-pelvic region, "duck walking", urinary disorders. Objective of the study. Improving the course of pregnant women with pelvic ring lesions during childbirth, due to mandatory orthopedic examination. Material and Methods. Analyzed the results of treatment 5 women with diastase of the pubic symphysis, after repeated births. Performed clinical examination, radiography, etc. Radiological examination determined diastase between the pubic bones 14 - 46 mm, and in 3 patients the ilio-sacral junction was enlarged to 8-12 mm. Study funded by NRDA, project no.20.80009.8007.11, 20.800009.8007. Results. In 3 patients performed reconstructive pelvic surgery in the early stages of labor, in 1 stage, in 2 patients the treatment in stages using osteoplasty, combined osteosynthesis. After reconstructive interventions, the patients noticed an improvement in the general condition, the persistent pain disappeared, the gait improved, the claudication disappeared. Patients returned to their previous level of physical activity within 4-6 months. Radiological control of the pelvis confirmed the effectiveness of the treatment; formed the consolidation, maintaining the stability of the metal structure. Results within 3-8 years: the value on the scale S.A. Majeed is 80-94 points. Conclusion. Ignoring orthopedic consultations by women after the first birth caused the formation of a more significant diastase of the pubic bones in subsequent births. Reconstructive interventions performed on an unstable pelvic ring give a positive result and is a method of choice. |
