Background: Threatening limb ischemia often develops in patients with multi-tiered atherosclerotic lesions. The combination of open and endovascular revascularization techniques offers the possibility of resolving several levels of lesions, at same stage without increasing the operation trauma. Objective of the study. To establish the benefits of hybrid interventions in limb-threatening ischemia. Material and Methods. Retrospective study within CRH "Timofei Moşneaga" in the period 2020-2021 in patients with threatening ischemia, treated using the hybrid method. The following aspects were analyzed: demographic data, comorbidities, types of interventions performed and their results. Results.Age range of patients 57-80 years. Comorbidities: Arterial hypertension, diabetes, cardiac insufficiency, angina pectoris, atrial fibrillation, cerebrovascular disease, chronic bronchitis, obesity. Types of interventions:Femuro-popliteal by-pass and iliac artery stenting -8 cases; endarterectomy and patch plasty of the common femoral artery and distal stenting-6 cases; common femoral endarterectomy, patch plasty and proximal stenting-8 cases; internal carotid endarterectomy/plasty and internal carotid stenting-3 cases. Mortality was 0%. Technical success - in 100% of cases. Major amputations up to 3 months - 2 patients. Conclusion. Hybrid interventions combine the well-established benefits of open vascular surgery permeability with the benefits of less invasive endovascular interventions to provide a complete, durable, and safe solution for patients with threatening limb ischemia. |
