Background. Fournier gangrene is a form of necrotizing fasciitis that affects the genital, perineal and perianal areas. Objective of the study. Analysis and establishment of the effectiveness of different methods of diagnosis and treatment of patients with Fournier's gangrene. Material and Methods. The paper presents the experience of Clinic 2 Surgery over a period of 19 years 2001-2020 on a group of 86 patients treated in the abdominal septic surgery department of SCR. The age of patients 19-66 years, with the prevalence of men. The diagnosis was based on data from clinical, laboratory and imaging investigations. Results. The causes of Fournier gangrene are: anorectal diseases 70 (87.5 %), urogenital diseases 8 (9,3%), soft tissue diseases 6(6,97%,) unidentified 2(2.3)%. Concomitant pathologies were recorded in patients with Fournier's gangrene in 56(65.1%) cases. Detected microbial germs: E.coli (47.1%), methyl resistant Staphylococcus aureus, Pseudomonas aeruginosa. The clinical picture has evolved in several consecutive phases. The diagnostic difficulties and the need to use contemporary imaging methods in the evolution of complications are determined. The practiced surgical treatment consisted in the stage surgical treatment of the septic focus (12-24h) and reconstructive operations. Conclusion. Fournier gangrene - is a major emergency, in which early diagnosis, surgical treatment and complex intensive care can improve clinical outcomes. |
