Background. Spinal tumor surgery is one of the most difficult branches of neurosurgery. The diagnosis of tumors in case of acute manifestations is quite difficult due to the similarity of the clinical picture with other neurogenic pathologies.Surgical treatment have often at major risk for neuronals structures. Objective of the study. To identify and appreciate of the microsurgical technique of excision of spinal tumors in the treatment tactics that would allow to improve the functional results of the surgical interventions. Material and Methods. Bibliographic synthesis of 58 articles addressing the problem of contemporary treatment of spinal tumors, using over 100 bibliographic sources, published during the years 2009-2019.) Results. For the total of 95 patients were included in the study. There were 56 men and 39 women with age average at the time of surgery being 45 years (18-64). Median time elapsed between the onset of symptoms and surgery was 12 months. The most common symptoms were pain 72%, sensory deficit 63%, and motor deficit 43%, decreased bladder function, decreased gastrointestinal tract function. Two patients underwent surgery at the level spine, and two were previously treated with chemotherapy.The most common histopathological diagnosis was ependymoma (73%, n = 69). Conclusion. I identified the optimal surgical method for the contemporary treatment of spinal tumors which is gross total resection because, at a correct total resection of the spinal cord, it is: the use of the posterior medial groove for medially located tumors. |
