Background. Modern phlebology has various techniques for surgical treatment of varicose veins, such as crosectomy, laser intravenous ablation, ASVAL and eASVAL methods, sclerotherapy, etc. Combining these methods is an opportunity in choosing the right and effective treatment option. Objective of the study. Optimizing the treatment of varicose veins of the lower limbs, by combining different surgical treatment techniques. Material and Methods. A prospective study was conducted during 2018-2020, including 80 patients with varicose veins of the lower limbs, 50 treated by EVLT of the large saphenous vein, in association with miniflebectomy and crosectomy, and 30 – by ASVAL or eASVAL methods, completed in 12 cases with sclerotherapy, with the preservation of the competent saphenous vein with segmental reflux. Results. The criteria for choosing the surgical technique was the presence of insufficiency of ostial valve, valves of the saphenous trunk with vertical venous reflux and the diameter of saphenous v. in the proximal part. Patients who underwent EVLT with crossectomy and mini phlebectomy had an insufficiency of both ostial and saphenous valves, with a diameter of saphenous v. >7-8 mm. In the II group the vertical venous reflux was slightly/moderate, with the same degree of impairment of valvular insufficiency and saphenous v. ≤7 mm. In group I, recurrences at 1-3 years were 4%, in group II - no recurrences, being established an improvement of saphenian blood flow in 91% of cases. Conclusion. Modern techniques for treating varicose veins have their degree of vigor, and their combination is most effective. ASVAL and eASVAL procedures combined with sclerotherapy are safe methods that can be performed separately in patients with segmental reflux in the saphenous vein |
