Background. Insulinomas are functional pancreatic neuroendocrine tumors with an incidence of 1-4 cases per million / population / year, being the most common form of this type of tumor. Objective of the study. Analysis of the sensitivity and specificity of diagnostic methods. Material and Methods. The paper presents the diagnostic results of 15 patients in the period 1993-2021 within the Clinic I of the Department of Surgery No. 2. The methods were based on clinical examination, laboratory investigations (glycemic profile, glycemia during hypoglycemic attacks, after administration of solutions 40% glucose, as well as the appreciation of the level of C peptide and serum insulin), USG, CT,RMN. Results. Clinical manifestations are: neuropsychic symptoms 13 (86.6%) cases, digestive 8 (53.3%) cases, adrenergic 10 (66.7%) cases, Cushingoid manifestations 3 (20%) cases. within the limits of 2.03.0 mmol / l, after soil administration. 40% glucose - 3.8-5.8 mmol / l. The average level of peptide C - 4.6 ng / ml, of insulin 32.45 μmol / l. Tumors with dimensions between 0.8- 2.5 cm with body location 10 (66.4%) cases and caudal 5 (33.6%) cases. Conclusion. The diagnosis of insulinoma remains a dilemma, based on clinical, laboratory and imaging examination (USG, CT, MRI). Currently, unanimously accepted imaging criteria are not. |
