Background. The prevalence of stroke in the adult population with a diagnosis of symptomatic carotid artery stenosis is estimated at 20-30%. Carotid endarterectomy and carotid artery stenting remain the most important therapeutic options in the treatment of this disease Objective of the study. Comparative evaluation of periprocedural efficacy and safety between endarterectomy and carotid artery stenting in patients with symptomatic carotid artery stenosis Material and Methods. The performed study represents a retrospective analysis of 450 patients treated within the IMSP SCR ,, Timofei Moșneaga ,, Department of Vascular Surgery during 2016-2021. The studied patients were divided into 2 groups: Group A - 250 patients undergoing carotid endarterectomy, Group B - 200 patients undergoing carotid artery stenting. Results. The rates of comparative efficacy and periprocedural safety in the patients under study were as follows: Lot A versus Lot B for death (1.6% vs. 0.5%, P = 0.18), for ipsilateral stroke (2.3% vs. 1.5%, P = 0.01 ) and for myocardial infarction (2% vs 1.5%, P = 0.03). At the end of the study period, in 2021 the ipsilateral ischemic stroke rate decreased in both study groups (0.4% and 0%, respectively; P = 0.85). Conclusion. The approach of patients with symptomatic carotid artery stenosis is a multidisciplinary one. Carotid endarterectomy in patients with symptomatic carotid artery stenosis is associated with a higher risk of periprocedural complications and death compared to carotid artery stenting. |
