Background. Left ventricular (LV) aneurysm is a serious mechanical complication of acute myocardial infarction (AMI) with an incidence of 10-35%. In most patients, it occurs due to late revascularization, although advances in interventional car Objective of the study. Study of postoperative and long-term results of aneurysmal LV reconstruction. Material and Methods. The study is retrospective comprising a group of 68 patients with post-infarction LV aneurysm operated in the period 2015-2020 in the Cardiovascular Surgery Clinic of the Republican Clinical Hospital. The type of ventricular reconstruction was: resection of aneurysm with linear suturing (Mickleboroughe), remodeling after Dor or after Jatane Results. The mean age of the patients was 59 years (40-80 years). The location of the aneurysms was: on the antero-apical wall in 85% of cases, apical in 10% of cases, posterior in 3% of cases, postero-inferior 2% of cases. Endoventricular thrombi were detected in 35% of cases. All patients had IC cl.f. III-IV NYHA, HTP 50-70 mmHg, mitral regurgitation gr III-IV, fibrous ring of dilated mitral valve 37-45 mm. In most cases, a coronary bypass was performed. FE clearly increased from 38% -59%, diastolic volume decreased from 254 ml to 170 ml, systolic volume from 112 to 80 ml. The regression of VM insufficiency was obtained up to grade I-II and the diameter of the fibrous ring was reduced Conclusion. Surgical reconstruction of LV (by remodeling techniques: Dor, Dor-modified, Jatane, Mickleboroughe), associated with myocardial revascularization, offers the possibility of restoring ventricular function, increasing global FE and decreasing end-diastolic volume |
