Background. Thyroid nodules are common lesions being highlighted clinically and by ultrasound up to 76 % in the general population. Due to the malignant character of the thyroid nodules is debatable the duration of their clinical and paraclinical observation and the opportunity of decision-making for surgery. Objective of the study. Optimization of tactics for diagnosis and surgical treatment of thyroid nodules. Material and Methods. C.L., a 45 years old female patient is complaining of discomfort in the anterior cervical region, "globus" sensation, irritability, and fatigue. She was under the endocrinologist's supervision for 7 years, the patient was diagnosed with a thyroid nodule in the right lobe, which despite medication with L-thyroxin 75 mg, increased in size. Results. Serum levels of thyroid hormones and TSH- without deviations. The ultrasound of the right lobe detected a nodular formation with hyperechoic elements surrounded by hypoechoic margin measuring 2,14x2,44 x3,25 cm, with active vascularization and mixed type. It confirms scintigraphically the presence of “hot nodule” at the upper pole of the lobe. Lobectomy was performed with the intraoperative frozen section, where was detected follicular carcinoma of the right lobe. The intervention ended with a total thyroidectomy. The diagnosis was confirmed by histopathological intervention, followed by long-term postoperative supervision without any complication. Conclusion. The evolution of thyroid nodules, without underestimating their malignant potential, requires a continuous clinical and paraclinical follow-up. With the appearance of suspicious signs of malignancy, the approach to thyroid nodules will be multidisciplinary and directed towards radical treatment. |
