Background. Maturity Onset Diabetes of the Young represent a monogenic form of diabetes mellitus, characterized by beta cell disfunction. The disease has autosomal dominant transmission. It is estimated that MODY diabetes accounts for 1-5% of all cases of diabetes in the young population. Objective of the study. Studying MODY diabetes to identify the particularities of diagnosis and treatment Material and Methods. Narrative study of the literature, identification of the most known forms of MODY, particularities of diagnosis and treatment. Results. As of today there are 14 types of MODY diabetes registred with different pathological mechanisms. The most known of therse types are HNF4-alpha MODY 1, GCK MODY 2 and HNF1-alpha MODY 3. Gene mutations of HNF4-alpha, GCK and HNF1-alpha, lead to the distruction of beta cells with a secretion disorder of insulin Genetic tests are the most important in MODY diagnosis. GCK mutation usually is causing a light asymptomatic hyperglycaemia. There is no need for special treatment for this type of patients.HNF4-alpha and HNF1-alpha mutations cause a deeper hyperglycaemia with vascular complications in time. In this case sulfonylureas are used in treatment. Conclusion. In the last decade, there has been a growing focus on monogenic forms of diabetes and their frequency in the population. The patient's medical history and laboratory data may be subjective for the existence of monogenic diabetes, but the most important method is genetic tests. |
