Background. Lung injury is the most common manifestation of COVID-19. SARS-CoV2 pneumonia often has an unpredictable evolution, with various imaging manifestations. Objective of the study. Evaluation of the clinical evolution and the terms of radiological resolution of moderate and severe SARS-CoV2 pneumonias. Material and Methods. The clinical, paraclinical and imaging data of 650 hospitalized consecutively patients with confirmed COVID-19 ( by PCR-RT) in pneumology ward of IFP „Ch. Draganiuc ”. Patients were examined according to the recommendations of the National Clinical Protocol, including chest radiography at hospitalization and after 8-10 days (and repeated at 18-24 days selectively). Results. In this group, the evolution of SARS-CoV2 pneumonia was severe in 38% of cases and non-severe in 62% of cases. At the radiological follow-up, at 8-10 days, in 93% of cases pulmonary consolidations was found and in 40% of the patients the progression of the radiological consolidation was noticed. Chest CT was performed in 50 patients after 1-3 months. In most patients imaging changes still were present: ground glass and / or reticular opacities, areas of fibrosis with traction bronchiectasis, pleural thickening. Imaging involution of lung consolidation was slower in patients with severe COVID-19 and / or a Brixia >10 on chest X-ray in the acute period of disease. Conclusion. SARS-CoV2 pneumonia has a slow resolution. Slowly resolving lung consolidation was documented even in non-severe pneumonia. Pulmonary sequelae are expected in patients who have experienced a severe form or in those with extensive lung damage. |
