Background. Rosacea is a chronic, recurrent dermatosis with a significant impact on patients' quality of life. At this time, there is no any well defined laboratory method which can confirm the presence of rosacea. This fact creates an impediment for the diagnosis and therapeutic tactics. Objective of the study. The aim of this research is to optimize the clinical-diagnostic and therapeutic management of Rosacea based on reference guidelines and protocols. Material and Methods. 11 guidelines for the diagnosis and treatment of Rosacea from Europe, North America and international dermatological associations were analyzed. The data from the last 7 years publications regarding the therapeutic conduct in rosacea, were examined and summarized. Results. The diagnosis of rosacea is based on the subjective assessment of the clinician, so it is important to use the scales to determine the severity of each phenotypic manifestation. More objective clinical and laboratory methods of diagnostic could be considered to be electronic dermatoscopy, doppler angioflowmetry and also the assessment of indices of hemnodynamics, but the value of these has not been established yet. Treatment of rosacea based on phenotype, using vasoactive drugs (Beta-blockers, Alpha-adrenergics) and Ivermectin, along with classical therapies, lead to better compliance and to an increase in the dermatological quality of life index of patients with rosacea. Conclusion. The choice of treatment options in rosacea should be guided by the phenotypic manifestation of the disease and new vasoactive and anti-inflammatory drugs, which act directly on pathogenetic chaines of rosacea, suggest an update of old standards, including the national one. |
