Background. Rheumatoid arthritis (RA) is considered one of the most common autoimmune diseases, it primarily affects the joints, extra-articular involvement can occur in about 40% of patients. Thyroid dysfunction has been reported in 6% to 34% of patients with RA. Objective of the study. Study of the clinical and laboratory characteristics of rheumatoid arthritis associated with thyroid gland pathology. Material and Methods. The information resources and scientific materials published in the specialized articles on the medical sites, the manual of Rheumatology and Nephrology and Harrisson of Internal Medicine were used. Results. People with RA are more likely to develop subclinical hypothyroidism at 37.9%, hypothyroidism 14%, only 0.3% hyperthyroidism and 47.8% euthyroidism. The most common cause of hypothyroidism was autoimmune thyroiditis (Hashimoto's thyroiditis), reported in 14.81% of patients. The presence of thyroid disorders in patients with RA has been significantly associated with female sex, anti-nuclear antibody (ANA) and anti-CCP positivity ≥ 100 EU / mL (citrulline anti-cyclic peptide antibodies). Conclusion. The presence of thyroid disorders in patients with RA suggests a more aggressive disease. Routine screening for serum thyroid stimulating hormone (TSH) is recommended in all patients with RA at the time of diagnosis and at a later annual interval. |
