Background. Cognitive impairments are common in patients with drug-resistant epilepsy and are associated with significant psychosocial dysfunction, increased mortality, and reduced quality of life. Objective of the study. to identify factors associated with cognitive impairment in adult patients with drug resistant epilepsy. Material and Methods. The present study, involved 51 patients with drug-resistant epilepsy, as evidenced by the National Center for Epilepsy. All patients were evaluated clinically, electroencephalographically and imaging (brain MRI), and tested neuropsychologically with MoCA test, Hamilton Anxiety Scale, and Beck Depression Inventory. Results. In 76.47% of cases, drug-resistant epilepsy is associated with cognitive impairment. Of this - only 20.5% have higher education. In 1/3 of the patients, no structural brain lesions were identified on MRI, but they present accentuated cognitive disorders (17.71; p = 0.05). Moderate and severe depression (in 28.2%) have a major influence on cognitive function (17.45; p <0.01). The onset of epilepsy in childhood and antiepileptic polytherapy (≥ 4) negatively influence cognitive function (p = 0.08). In the case of drug-resistant epilepsy, the presence or absence of interictal epileptiform discharges and anxiety do not correlate with the level of cognitive impairment. Conclusion. Educational level, polytherapy with more than 4 antiepileptic drugs, depression and unidentified of a structural lesion on neuroimaging are predictive factors of cognitive impairment in patients with drug resistant epilepsy |
