Background. Hepatitis G virus (HGV) in children with malignant tumors can be transmitted parenterally, with the development of acute hepatitis and severe acute liver failure. Objective of the study. To determine the cause that triggered severe acute liver failure in a patient with nephroblastoma, undergoing radical surgical treatment, polychemotherapy and radiotherapy. Material and Method. The study included data from the 18-month-old inpatient medical record, diagnosed in December 2019 with "Right kidney nephroblastoma. St III.Nx.M0". Results. Child diagnosed at 13 months,performing over 5 months of nephrectomy on the right, with intraoperative tumor rupture and cholecystectomy.Pre/postoperative liver function not altered. Neoadjuvant PChT, with 70% tumor reduction.Radiotherapy.Adjuvant PChT. 1,5 months after treatment and 2 adjuvant PChTmucocutaneous jaundice, severe cytolysis, hemolysis, acute hepatic and renal failure.Treatment in ATIsymtomatic/ hepatoprotective,reduction of PChT doses partially stabilized the child,failure of acute hepatic and renal,but cytolysis persists.Examination of HVB DNA and RNA HCV,RNA VHD,RNA VHG with analytical sensitivity <5 IU / ml,confirmed HGV. Conclusion. Mechanism of hepatotoxicity and acute liver failure was induced by tumor endotoxinemia, specific treatments as well as parenteral HGV contamination. Dynamic monitoring and antiviral therapy with alpha 2beta-interferon will influence the prognosis of the disease. |
