Background. From the data of M. Moattari and coauthor. (2018) nerve injuries in associated open hand injuries are 23.7%, of which 76.8% are associated with tendon injury. Digital nerves damage is given minor attention, often being ignored when restoring anatomical integrity. Objective of the study. Analysis of surgical treatment results in associated lesions of the peripheral nerves of the upper limb. Material and Methods. Clinical study based on the treatment of 83 patients with associated nerve injuries with flexor tendons of the thoracic limb. The lesion of the median or ulnar nerve was in 58 cases, 23 being with the lesion of both nerves. In 12 cases the lesions of the common and proper digital nerves were detected in 13 cases. Epiperineural sutures were applied in 79 cases and epineural sutures in 4 cases. Results. The remote viewing were followed in 69 cases (83.13%). Good results were recorded in 51 cases, satisfactory in 27 and unsatisfactory in 5. To obtain good results it is necessary to optimally and symmetrically adapt the ends in neurorrhaphy, which is obtained only by applying the microsurgical technique, optically increasing the operating field. Technique used by us in 95.18% of cases, when epiperineural sutures were applied. Unsatisfactory results were in 4 cases, when were applied epineural sutures at the first stage without microsurgical technique. The recovery in associated nerve damage depends on the correct assessment of the indications for the application of primary sutures. Conclusion. In the associated lesions of the peripheral nerves, neurorrhaphy is mandatory when performing the primary surgical toilet. In order to restore function, it is necessary to avoid vascular lesions of the area and to adapt the ends optimally and symmetrically using microsurgical techniques. |
