Background. The frequency of closed pelvic injuries, from isolated cases, reached 7-10%, and continues to grow. The treatment of pelvic fractures in polytrauma remains difficult. The reasons are uncontrolled blood loss, late pelvic stabilization, the development of early posttraumatic complications. Objective of the study. Improvement of the treatment in polytrauma patients with pelvic fractures based on the use of minimally invasive devices and techniques in the initial period of traumatic disease. Material and Methods. Over 30 years, more than 718 patients with multiple and associated injuries and pelvis fractures were treated in our clinic. They were examined by clinical, laboratory and instrumental methods. Closed pelvic osteosynthesis was performed. Understanding of the fracture type allowed the authors to develop several new fixation devices and stabilization methods. Results. Treatment results were evaluated by Majeed scale. Satisfactory reposition of the pelvic ring and stable fixation were achieved in the main group of patients (82%) in the operating room. Patients mobility was restored after osteosynthesis, active regime was intensified. After 14-21 days, they were transferred to outpatient treatment. Fixation period of the pelvic ring in patients with type A and B fractures was 62±1.4 days, in type C fracture - 66.4±1.8 days. Analysis of treatment outcomes indicated that in 67.53% of cases good results were obtained, in 29.87% - satisfactory. Conclusion. Development and phased improvement of effective surgical treatment methods based on the principles of early external fixation and minimally invasive medical technologies provided positive results in 97.4% of polytrauma patients with pelvis fractures. |
