Background. Bezold abscess is a complication of suppurated acute mastoiditis and is a purulent collection between the posterior ventricle of the digastric muscle and the sternocleidomastoid muscle. In the age of antibiotics, this pathology has become extremely rare. Objective of the study. Presentation of a clinical case, with rare pathology - Bezold abscess, hospitalized and treated in the ENT clinic of SCR. Material and Methods. Male, 41 years old presented with: Otorrhea 20-day, AD hearing loss, right cervical swelling, odynophagia, low-grade fever (37.4 ° C). History of untreated otorrhea, chronic alcoholism, alcoholic polyneuropathy. Objective examination (O), Otoscopy (Ot), Oropharyngoscopy (Or), Schuller's radiography (R) were performed. Results. O: infiltration, cutaneous hyperemia, fluctuation,oedema in the region of the mastoid apex and right cervical tissues; hemodynamically stable; Ot: AD oedema, maceration, narrowing of EAC, abundant pus secretions, MT is not visualized; Or mucosal hyperemia, right parapharyngeal swelling. R opacification and lack of right mastoid pneumatization. Diagnosis: Chronic suppurative otitis media AD. Acute mastoiditis.Bezold Abscess. Emergency total mastoidectomy and lateral cervicotomy were performed . 400ml of pus was drained. 3Antibiotics from different groups. Suturing on the 8th day. Discharged on the 11th day. The sutures were extracted at the place of residence. Control in 2 months. Conclusion. Bezold's abscess is a very rare complication of late-diagnosed mastoiditis. Mastoid pneumatization predisposes to the pathology given by thinning and erosion of bone walls. Surgical treatment and broad-spectrum antibiotic therapy are essential for healing. |
