Background. Cancer of the extrahepatic bile ducts and papilla Vater occupies the 20th place (0.4% o) in the structure of oncological morbidity in the Republic of Moldova. Aggressive evolution and late diagnosis are argued by increased mortality, which represents 10.5%o. Objective of the study. To assess the stage of the tumor and the volume of surgery in extrahepatic bile duct and Vater papilla cancer. Material and Methods. The retrospective study includes 38 patients with malignant tumors of the extrahepatic bile ducts and Vater papilla under the Diagnostic Consultative Center of the Oncological Institute during 2016-2019, with a mean age of 62.7. Results. Evaluation of the site of the primary tumor detected proximal bile duct cancer (Klatskin's tumor) in 44.7% of cases; Vater's papilla cancer - in 42.1% and choledochal cancer - in only 13.2%. At the stage of establishing the diagnosis, regardless of location, the advanced stages were predominantly recorded in 30 cases (stage III - 23.7% and stage IV - 55.2%), which underwent only palliative treatment, at the same time stage I and II was assessed in 8 cases (21.1%), in which radical surgical treatment was possible. Conclusion. The late diagnosis in the advanced stages, which constitutes 78.9%, determines in the vast majority of cases the application of palliative surgical treatment, which corresponds to the current statistical data. |
