Background. Brain damage in autoimmune pathology can be manifested by seizures. In Multiple Sclerosis (MS) this phenomenon, as a primary manifestation, is quite rare. Likewise, in Systemic Lupus Erythematosus (SLE), this manifestation is not so common; seizures sometimes precede the diagnosis. Objective of the study. To perform the differential diagnosis (DD) between MS and SLE, having as primary manifestation seizures and cerebral demyelinating process. Material and Methods. Clinical case of the woman with primary symptom – seizures and demyelinating process on brain MRI. Also, the publications were searched by keywords on PubMed and were analyzed. Results. In women of fertile age, through demyelinating syndrome, SLE can mimic MS clinically (like-seizures) and on imaging. Relevant data for DD, using the symptoms and imaging (review), were not reported. The detection of oligoclonal bands in CSF was not shown to be specific, in some patients (review) with MS they were absent, and in SLE they were reported as positive in 15-80%. Immunological testing of antinuclear antibodies and patient monitoring over time could be effective methods for DD. Conclusion. In women of fertile age seizures as a primary manifestation, could denote cerebral autoimmune pathology, characterized by demyelinating process. DD between MS and SLE would be the use of specific immunological tests and patient monitoring over time. |
