Background. Malignant lymphomas account for 3-4% of all cancers. Peripheral nerve damage occur prior to the clinical experssion of lymphoma or develop over time. Electrophisiological evaluation is mandatory in any any neuropathy of obscure etiology where lymphomas are placed for differential diagnosis. Objective of the study. Elucidation of clinical and electrophysiological peculiarities of neuropathies associated with Hogkin and Non-Hodgkin's lymphoma Material and Methods. Clinical neurological examination, electrophysiological examination, CT or MRI study and lumbar puncture were performed in 83 non-Hodgkin and 47 Hodgkin lymphoma patients, morphologically confirmed. The descriptive statistics were used for data analysis. Results. Age ranged from 16 to 78 years. Women were three times more likely to be affected than men. Clinical impairment of peripheral nerves was reported in one third of examined patients. Polyneuritic "socks-gloves" alteration was recorded in 37.3% of cases. Vibration sensitivity was altered in 89.6% cases: in all four limbs in 56.6% of non-Hodgkin lymphoma patients and in none of Hodgkin lymphoma group. The lower limbs vibration was reduced in half of joint patients group. The abolished Achilles reflex was detected in 39.7% of casesMost cranial nerves alterations occurred in non-Hodgkin's lymphoblastic lymphoma. Multiple cranial nerve lesions were registered in 27 % of the cases. Conclusion. There is a certain clinical and electrophisiological correlation of peripheral nervous system alterations associated with Hodgkin and non-Hodgkin lymphomas. Accurate clinical assessment combined with electrophisiology exam facilitate the early diagnosis and interventions in malignant lymphoma. |
