Background. WHO estimates in 2020 1 million children with tuberculosis (TB), 25000 with resistant TB, only 10% being diagnosed and treated. Resistant TB in children is primary. The diagnosis and treatment of TB in children is difficult. Objective of the study. Studying the impact of risk factors and the peculiarities of the evolution of resistant TB in children. Material and Methods. S-a efectuat o cercetare retrospectivă descriptivă evaluînd foile de observație a 206 copii cu TB rezistentă internați pentru tratament în Spitalul Clinic Municipal de Ftiziopneumologie din Chișinău în perioada de timp 2009-2018. Results. Age predominated ≥ 15 years - 92 (45%). Detected by active screening 157 (76%) patients, passive case finding - 49 (24%). The majority 184 (89%) are from contact clusters, among them 125 (68%) with resistant TB. It was diagnosed with intrathoracic lymph nodes TB 77 (37%), primary TB complex - 32 (16%), infiltrative TB - 91 (44%), pleurisy - 6 (3%). IDR 2 UT resulted in normoergic reaction 118 (57%), hyperergic - 66 (32%), negativ - 22 (11%). Drug resistance, confirmed by culture positive test in 138 (67%) cases, in 68 (33%) - empirical treatment was administered. Positive treatment outcome resulted in 100% cases, recurrence registered only by reaching adulthood. Conclusion. Resistant TB in children reflects transmission of a resistant strain from an adult contact source. Confirmation microbiological resistant TB in children is a dilemma. Children tolerate antituberculosis drugs well and the result of treatment is positive. |
