Background. Cardiac remodelling is one of the pathogenic pathways leading to heart failure, which occurs due to ischemic, mechanical and inflammatory damage to cardio myocytes and cardiac interstitium. Objective of the study. Detection of biochemical markers of cardiac remodeling in the context of ischemic myocardial injury. Material and Methods. Have been analysed 82 bibliographic sources published during the 2000-2019 in the electronic databases Medline, PubMed, Medscape, Hinari and Google Academic, as well as from the Medical Scientific Library of “Nicolae Testemitanu” State University of Medicine and Pharmacy. Results. The cardiac remodeling promotes an increase in serum levels of proinflamatory cytokines (TNF-alpha, IL-1, IL-6, TGF-beta), vasoactive peptides (angiotensin II, endothelin-1, natriuretic peptide B), and lecithins (Galectin-3). Recent studies have mentioned the diagnostic value of plasma levels of galectin-3 as a marker of myocardial fibrosis in patients with heart failure. It has been shown that galectin-3, secreted by activated macrophages, through its paracrine and autocrine action induces the activation of pericytes, myofibroblasts, macrophages and interstitial fibroblasts, cell proliferation and the synthesis of type I procollagen. Conclusion. Early detection of pathological heart remodeling reduces the risk of heart failure, and death. It is recommended to evaluate galectin-3 and natriuretic peptide B as early markers of pathological cardiac remodeling. |
